& Checklist of Required Documents to Bring to AFA
to Complete Your Child’s Enrollment

Please do the following steps in order to ensure completeness & ease in finishing enrollment for each child you intend to register at Al Fatih Academy.

  a) Complete the registration form online.
  b) Print out completed registration forms for each child.
  c) Please come at your assigned time to Al Fatih Academy’s office to complete the final steps of the enrollment process. 

 

[Note that different appointment times may be given if you have an additional children registering for different grades.]


  d) Please bring the following items for your registration to be valid at AFA:

  i. The paper copy print out of the completed registration form

If new students enrolling into AFA for the 1st time, or

  ii. Official documentation of your child’s birth date, ie the original form of:

  1. birth certificate, or
  2. passport

  iii. Please bring your up-to-date Parent Volunteer Hours Log form with the cumulative totals for each parent/guardian.

  iv. Your checkbook to write separate checks for each individual student being enrolled:

  1. One check for AFA’s registration and supply fee of $175 (non- refundable).
  2. A 2nd, separate check for the first month’s Tuition Fee or 1st annual or 1st semi-annual payment (non- refundable).
  3. If half of your Parent Volunteer 20 Hours are still incomplete -- ie 10 hours per family --or the Log is lost/forgotten, a 3rd, separate check for the remainder balance due.

  v. For a newly enrolling student who has attended another or more school/s, please bring copies of all previous academic records.

  vi. If any of the descriptions below apply, please bring your child’s health assessment form INCLUDING their latest immunization record [Virginia’s official blue-papered form]

  1. new students enrolling into AFA for the 1st time, or
  2. a returning AFA student who is a rising Kindergartener,
  3. a returning AFA student with updated immunizations or health changes


Health assessments and immunization records
must be received in AFA’s office by 1 August 2007.

Per Fairfax County’s licensing requirements,

your child will NOT be allowed to attend class
 until AFA has your child’s LATEST health assessment & immunization records on file.
